* Data form completed and signed by legal guardian
* Proof of legal residence (water bill, gas bill, electric bill, or rental / lease agreement)
* Proof of immunizations
* Birth certificate
* Social Security card
* Proof of current physical (applicable only for first time admission in Robertson County)
* Home language survey form filled out
* Custody papers (if applicable)
* Proof of appropriate grade placement (transfer slip preferred) current report card may be acceptable
* Release of records request signed


Note: Your child must turn 5 years old on or before September 30 to enroll in Kindergarten this school year.
